Sign up for a 20-minute assessment between 5:30 - 7 p.m., for:

Athletico cannot provide free screenings for federally funded health care plans.
People need to call Athletico for an appointment at (217) 352-3330 to reserve a slot. Walk-ins are also accepted. This is for people ages 15 and up.
